"How can this work if the player holding an amulet is voted out?"
I assume if a player is voted out holding an Amulet it is then considered "an Amulet NOT remaining in the game" so then only the remaining 2 Amulets have to be played together (or 1 Amulet played as a Regular Immunity Idol).
"If you have to play all three at once how can an amulet become an idol?"
You only have to play all three at once if all players holding an Amulet are still in the game then all Three of you must Play your Amulets together as a Single Extra Vote.
Or if one of the players holding an Amulet was voted out then the Remaining Two Players must play their Amulets together as a Steal-A-Vote.
Last there are special rules for One Amulet remaining...

This has been discussed a lot on the internet. Lots of people find the phrases:

To mean: **IF** you want power your Idol, the other 2 idols must also be found and IF you want to know here is how you will know"
To me it is clear that originally they meant you MUST say your Secret Phrase at the next Immunity Challenge.
Evvie has confirmed that Xander asked Production if he has to say his Secret Phrase and Production originally said YES at EVERY Immunity Challenge.
Production later changed their mind and told Xander he could wait to hear one of the other Secret Phrases and then he MUST reply with his Secret Phrase.
Apparently, Production this season has made it totally optional maybe after Daniel or Mike asked if they MUST say the Secret Phrase at the NEXT Immunity Challenge.
